 Service Level 4 - Insurance Authorization Requests

Prior completion of Service Level 1 is required to assist you. We will draft a letter, minus letterhead, and forward to your physician for his edit or submission to your insurer, (usually necessary for insurance coverage). Letter of Request will take account of your insurance company’s policy for genetic testing, pertinent family health history and family structure information, your own clinical information, professional society guidelines, literature reviews and risk data, as needed to seek approval. Our success rate is extremely high in obtaining insurance coverage for genetic testing. Both client and physician should receive notification from insurer of coverage decision.
Please refer to the Forms tab in the main menu, for a listing of forms that are required for this Service Level. A link to the forms, which can be submitted online, will be forwarded to you with verification that we have received your payment.
