  • For new companies preparing to enter the Genetic testing space, up to 3 Corporate team members including C-level executives, founders, and key scientific staff

Genetic Testing Informed Consent or Results DisclosureGenetic Testing Informed Consent or Results Disclosure
  • This service provides Informed Consent before genetic testing or Test Results Disclosure and explanation after testing.  
  • A board certified, licensed Genetic Counselor will provide a Consultation note to you and your referring provider which will facilitate medical management. 
  • Allow 30 to 40 minutes for discussion.
Personalized Risk AssessmentPersonalized Risk Assessment
  • This service provides Genetic Counseling and Risk Assessment for patients and families concerned about cancer.
  • This service provides Genetic Counseling for other genetic conditions, suspect genetic conditions, for pre-conceptional concerns, prenatal concerns, neurologic, hematologic, cardiac and other conditions in the family history.
  • Service can be used for professional review of test results. 
  • If you seek genetic counseling for Direct to Consumer Ancestry or 23&me Genetic Testing, please go to Customized Requests.    
  • One hour consultation. Please complete all forms online, and submit any pertinent records by fax (888-204-5975) prior to appointment.  

Service Level 1- Initial AssessmentService Level 1- Initial AssessmentService Level 1 - Initial Assessment. This is the best place to start. We will review your family history , answer many of your questions, discuss testing eligibility and tell you which Services most apply to you.  This service can be requested for full risk assessment, of 60 minutes, in expedited cases.  30 or 60 minutes appointment.

 Initial Assessment can also be used to collect a full Pedigree for later use in genetic counseling or otherwise. 30 minute appointment required for this option.   
Service Level 2 - Medical Record RetrievalService Level 2 - Medical Record RetrievalAlthough your doctor can readily (and at no cost) provide many medical records, there are sometimes critical records which may be difficult to obtain. We can help in locating older records, records on relatives or laboratory specimens for testing. This service is free to established clients whose insurer or clinic is contracted with us. Otherwise, charge per record.
Service Level 4 - Insurance Authorization RequestsService Level 4 - Insurance Authorization RequestsWe will draft a letter of necessity, providing key information about the benefits of genetic testing for your health care-- for you or your physician to use, edit or include in efforts to support your medical necessity for genetic testing. We will assist with forwarding to your insurer directly or through your physician, depending on the requirements of your insurer and your preferences. If your insurer is contracted with a genetic testing laboratory that offers genetic counselor assistance with testing authorization, you won't need our help, and we will help you to figure that out, before doing anything.

Please note that genetic testing surrounding on-going prenatal or pediatric diagnoses is best handled by  Maternal/Fetal, Obstetric, and Pediatric Genetics Specialists, often located at major medical centers or hospitals,  where clinical/medical services may be offered if needed.
